858 Parathyroid Adenoma Manifesting as Acute Pancreatitis: A Case Report and Literature Review

Abstract Introduction Pancreatitis is commonplace on the acute surgical take. It usually caused by gallstones or alcohol excess. Parathyroid adenoma causing hypercalcaemia a rare cause of pancreatitis. Case report Here we describe case 45-year-old gentleman with no past medical history who presented epigastric pain and vomiting. A CT scan revealed After normal biliary tree ultrasound scan, was initially proposed to be although he reported an average intake only 4–6 units/day. However, closer inspection blood tests, it apparent that had severe at 3.45mmol/L hypophosphataemia. This raised suspicion primary hyperparathyroidism. Accordingly, parathyroid hormone level measured shown 90pmol/L! An neck confirmed underlying adenoma. Due severity electrolyte imbalances, this excised during same admission. The patient's symptoms resolved, discharged planned follow up after genetic testing for multiple endocrine neoplasia syndromes. Literature review There have been many similar cases in last 10 years but hyperparathyroidism pancreatitis remains rare. Multiple involved recurrent before lesions were diagnosed. Conclusion highlights importance investigating other causes where excess ruled out. Furthermore, pertinent trainees, check tests initially, able interpret imbalances context disease.
